The most important message I would give clinicians is not to view nonadherence as the enemy. First-episode patients are going to stop their medication. Rather than fight it, I have learned to work with it, meaning that I try to find ways to help patients stay on their medication. If you can’t do that, then you need to find ways that you can maintain engagement and an alliance with the patient so that the consequences of nonadherence are less devastating.
